Region 5 Systems is seeking a Case Manager, known as a Professional Partner, to support youth, young adults, and families in southeast Nebraska.

Case Manager / Professional Partner

This is a remote-based position located in Lincoln, Nebraska. Employees work from their residence and perform most client-related field work in person within the communities served by Region 5 Systems in southeast Nebraska.

What You’ll Do

  • Provide wraparound case management services to youth and young adults under age 26 with serious emotional disturbances.
  • Support youth at risk of out-of-home placement, school disruption, or juvenile justice or child welfare involvement.
  • Develop individualized, strength-based plans with participants and families.
  • Coordinate services with schools, community providers, and support systems.
  • Support youth transitioning from the youth behavioral health system into adult services.
  • Maintain documentation according to program guidelines.

What You’ll Have

  • Education: Bachelor’s degree in human services or a related field.
  • Recent and soon-to-be graduates are encouraged to apply.
  • Three or more years of related human services experience may be substituted for a degree.
  • Preferred: Experience working with individuals with serious emotional disturbances or behavioral health direct service experience.
  • Ability to work effectively in a remote environment using video conferencing, electronic records, shared documentation, and network drives.
  • Strong communication, facilitation, documentation, prioritization, and coordination skills.
  • Reliable personal transportation and satisfactory background investigation required.

Region 5 Systems is one of six Behavioral Health Regions in Nebraska and is responsible for the development and coordination of publicly funded behavioral health services across a 16-county service area in southeast Nebraska.
